When people think about organ donation and transplantation, they often picture highly specialized transplant centers, cutting-edge technology and some of the most advanced medicine in healthcare.
What many don’t realize is that organ donation itself is a low-frequency event.
While that distinction may seem subtle, it has implications for how donor information is captured, shared and evaluated.
Across the United States, donor hospitals are spread across thousands of communities. While transplant centers perform organ transplants every day, most donor hospitals encounter organ donation only occasionally. In fact, many donor hospitals see just a handful of donor cases each year.
That reality creates a challenge that is often overlooked: healthcare systems designed for high-frequency events don’t always work well in low-frequency environments.
An Overlooked Challenge of Donor Hospitals
Organ donation doesn’t happen in a small number of specialized facilities. It happens wherever potential donors are identified.
According to CompuMed’s analysis of SRTR Donor hospital data, of the 6,000 hospitals in the U.S., only about 2,000 are a donor hospital in any given year.
“About 50% of these donor hospitals have three donors a year or fewer,” says Lee Keddie, CEO of CompuMed. “Roughly 80% average fewer than one donor a month.”
The result is a classic “long-tail” distribution in which cardiology, pathology, pulmonology and advanced imaging expertise is needed everywhere, but individual hospitals may rarely encounter donor-specific workflows.
This creates a challenge that is often overlooked.
“For each donor event, you need multiple subspecialty experts available any time of the day, any day of the week, any place across the nation,” says Keddie. “But the actual events when spread across the donor hospitals are incredibly infrequent. And even the total volume is very low. Those same 5000 echo interpretations would only provide a single cardiologist enough work for 50% of their time. It’s a real conundrum.”
Why Traditional Solutions Fall Short
Historically, healthcare technology has been designed for high-frequency environments.
Systems that require extensive setup, dedicated infrastructure or ongoing training can work well when they are used every day. They become much harder to implement when a hospital may only encounter a handful of donor cases each year.
That’s one reason organ donation has often relied on phone calls, screenshots, emails and manual workarounds. The reality of donation doesn’t always align with the assumptions built into traditional healthcare technology.
Keddie believes that disconnect is fundamental.
“Even though transplant is high frequency, highly specialized and supported by significant resources, organ donation itself is a low-frequency event,” says Keddie. “You can’t set up processes the same way you would for high-frequency events.”
That’s why some industry leaders are taking a new approach – to design tools and services around the realities of donor hospitals, rather than assuming every facility has constant exposure to donor management workflows.
Moving Data Instead of Moving People
The solution is not necessarily putting more specialists in more locations.
As CompuMed sees it, the future is about moving data, not people.
“Any kind of tools that can allow the subject expert to be separate from where the subject matter is create opportunities to bring expertise to donor hospitals without requiring specialists to be physically present,” says Keddie.
